The intake process at Center for Pain Control is timely and convenient. Once you make your initial appointment, please complete and submit our online Patient Intake Forms below. As an alternative, you may download, print, and complete the forms prior to your arrival and bring it with you to your appointment. When you arrive at our Wyomissing, Pennsylvania pain management facility, simply check in with our receptionist and relax in our welcoming atmosphere.

For workers’ compensation cases, we require the following information: date of injury, diagnosis code, employer address and phone number at the time of the injury. Please be sure to include this information when you complete your forms. This information can be obtained from the "Notice of Compensation Payable" form, which your employer or insurance carrier should have provided you at the time of injury.
